Physician and an Advanced Practice Provider (APP)

The physician and advanced practice provider (APP) work hand-in-hand to give you the best coordinated care possible along with nurses and other important healthcare professionals.

Licensed Clinical Social Workers (LCSW)

• Assess, diagnose and provide treatment for mental, emotional and behavioral Issues
• Provides Psychotherapy
• Conducts Crisis Assessments

Community Health Workers

• Provides one-on-one support in convenient locations
• Informs of available community resources
• Assists in overcoming barriers with food, housing or transportation

Community Paramedic

• Manage patient’s health at home
• Avoid recurring hospital and emergency room utilization
• Avoid recurring EMS utilization
• Facilitate communication with patient’s healthcare team

Pharmacy Team

• Pharmacist available on-site for personalized support
• Help with drug insurance coverage and plan questions
• Expert answers to medication-related concerns
